OTRS message - exception on TER website

Hi,

I don't know who to ask, typo3.org hardly helps finding an appropriate mail address.

Last week I created a new extension key (form4_syslog_report) and since than the three buttons "Delete", "Edit" and "Upload" lead to an exception - the typical "Oops, an error occurred!"-page.

On clicks nothing happens.
